When you try to talk about your health to someone, is it really difficult? What could be better?
We want to understand how services could be changed to make access, care and treatment fair for everyone, regardless of a long term health condition, your age, the language you and your family speak, ethnicity, faith, sexuality, gender, disability, or anything else that makes you unique.
The survey
This survey asks questions about how health and care services can improve, how the NHS could talk to you about healthy lifestyles in a way that works for you, and asks about the communication barriers that you may face.
It should take around 10-15 minutes to complete.
Everything you tell us is treated confidentially. We understand that we're asking you to share some sensitive and personal experiences with us in this survey. We're grateful that you are taking the time to tell us your story.
Everything you tell us will be used by our local Integrated Care System to reduce barriers and solve the problems you face when trying to look after your health.
